Dextromethorphan (DXM) Drug Test Dip Card

Dextromethorphan, commonly known as DXM, is a cough suppressant often found in a wide range of non-prescription cold and flu remedies. When used according to recommended dosages, it may cause only mild side effects such as dizziness, nausea, sweating, fever, vomiting, or pupil dilation.

However, DXM has become a substance of abuse, particularly among teens and young adults. When consumed in large amounts, it can produce serious and potentially dangerous effects including confusion, seizures, agitation, hallucinations, disorientation, and loss of coordination.

Though DXM is not typically detected on standard drug panels, it can trigger a false positive for PCP (Phencyclidine) due to chemical similarities.

Why Test for DXM?

Abuse of DXM is growing rapidly, especially among younger populations, due to its accessibility and hallucinogenic properties, which can mimic those of PCP. In emergency medical settings, patients experiencing seizures without a clear cause are often evaluated for DXM levels, and some may require referral to substance abuse treatment programs.

There are instances where DXM is mixed with drugs like heroin to intensify its effects.

Since DXM is commonly available in household medicines, misuse can go unnoticed—an empty cold medication pack or cough syrup bottle may not raise immediate concerns for many parents.

However, ingesting large quantities of these medications can be extremely harmful and has led to thousands of ER visits annually. Risks include liver failure, stroke, heart complications, coma, and even death. If you believe your child or teen may be misusing cough medications, it’s crucial to seek medical attention right away.

About Dextromethorphan:

Commonly available in over-the-counter medications, DXM is intended to treat cough symptoms, but in high doses, it can lead to dissociation, euphoria, and cognitive impairment. Abuse is rising due to its easy availability and low cost.

Relevance in Testing:

Quick urine tests can detect DXM within 1–2 days, making them ideal for routine screening in occupational health, rehabilitation, and drug court settings. Monitoring helps confirm responsible use, support recovery, and prevent reoccurrence of substance misuse, especially in populations at higher risk.

How to use a Dextromethorphan Dip Test

Remove the dip card from the sealed foil pouch. Then, remove the cap from the dip card.

1. 1. Immerse the exposed absorbent test strips of the dip card into the collection cup urine sample for 10-15 seconds.
   2. Allow the urine sample to migrate up the drug test strip.
   3. Replace the cap over the exposed absorbent test strips of the dip card and lay the dip card flatly on a non-absorptive clean surface. Wait for 5 minutes.
   4. Finally, read the results at 5 minutes. Do not read after 10 minutes.

How to Read Dextromethorphan Drug Test?

Positive

Only one colored band appears, in the control region ©. No apparent colored band appears in the test region (T)

Negative

Two colored bands appear on the membrane. First brand appears in the control regions © and another band appears in the test region (T).

Invalid

Control band fails to appear. Results from any test which has not produced a control band at the specified read time must be discarded. Please review the procedure and repeat with a new test. If the problems persist, discontinue using the kit immediately and contact your local distributor.
